Medication

Perry Hill Elementary personnel are prohibited from providing, or administering any medication to a student, except as authorized by the following procedures:

• The medication must be in the original container with the pharmacy label intact.
• The parent must complete a Medication Permit Form
• Indiana State Law requires that any unused medication for students in grades K-12 be sent home only with a parent, or a designated person over the age of 18. This designation must be in writing and on file in the school office.
